The outcome of a Person Stop Smoking Episode at the 4 week follow-up after the SMOKING QUIT DATE on a self-reporting basis. That is those who have received at least one session of a structured, multi-session intervention (delivered by the NHS or one of its trained agents), whose quit status at four weeks from their SMOKING QUIT DATE (or within 25 to 42 days of the SMOKING QUIT DATE) has been assessed (face-to-face, by telephone or by postal questionnaire).

National Code 1 'Successfully quit at 4 week follow-up (self-report)' should be those who report not having smoked since 2 weeks after the SMOKING QUIT DATE.

National Code 3 'Non-smoking status not known/lost to follow-up' should include all PERSONS whom it has not been possible to successfully follow-up at four weeks (-3 days or +14 days) from the SMOKING QUIT DATE.

For follow-up, PERSONS should either be seen in-person or contacted by telephone. Up to 3 attempts should be made to reach the PERSON by telephone, text or email; any PERSON not contacted after 3 attempts should be assumed to be 'lost to follow-up'.
