Newborn Hearing Screening is recommended to be undertaken on all newborn babies, using appropriate electro-acoustic or electro-physiological tests to detect moderate, severe or profound permanent childhood deafness.

The NHS Newborn Hearing Screening Programme (NHSP) ensures all parents are offered hearing screening for their new child within the first few weeks of life. It is a core service within the NHS in England and part of the family of Antenatal and Newborn Screening Programmes.

A PATIENT is a PERSON.

A PATIENT is a PERSON with a specific disease or condition who receives treatment from a Health Care Provider or any REGISTRABLE BIRTH.

It is an entry on the PATIENT master index.

This will be a PERSON, which includes neonates (babies aged 28 days or less), who use a Hospital Bed in order to receive clinical care/treatment or someone attending an Out-Patient Clinic, Day Care Facility, etc.

It will also include people in the community receiving care under a specific NHS SERVICE AGREEMENT forming part of 'nursing care in the community'. This also includes PATIENTS on the ELECTIVE ADMISSION LIST who are awaiting Elective Admission.

National Codes:

The ACCOMMODATION STATUS CODE of PATIENTS with any mental disorder should be captured periodically, typically as part of the PATIENT's regular Care Programme Approach Review. The data item should be based on the PATIENT's main or permanent residence.

Usage in the CDS:
The Outpatient and Future Outpatient CDS Types use the APPOINTMENT DATE as the "CDS ORIGINATING DATE" as a mandatory requirement of the CDS Exchange Protocol, see CDS ACTIVITY DATE.

For the Future Outpatient CDS where no APPOINTMENT DATE is available from the healthcare system, a default date value of 2999-12-31 may be applied.

Care must be taken to generate the correct CDS Exchange Protocol when using this default value.

When submitting a Referral To Treatment Clock Stop Administrative Event via the CDS V6 TYPE 020 - OUTPATIENT CDS, APPOINTMENT DATE is equivalent to the REFERRAL TO TREATMENT PERIOD END DATE carried in the record.

Consultant Clinic is a CLINIC OR FACILITY where the CLINIC OR FACILITY PURPOSE CODE is National Code 01 'Clinic', and where the care professional responsible for the clinic is a CONSULTANT.

Nurse Clinic is a CLINIC OR FACILITY where the CLINIC OR FACILITY PURPOSE CODE is National Code 01 'Clinic', and where the care professional responsible for the clinic is a NURSE.

ATTENDANCE DATE is the same as attribute ACTIVITY DATE where the ACTIVITY DATE TIME TYPE is National Code 33 'Attendance Date'.

A sequential number or time of day used to enable an attendance to be uniquely identified.ATTENDANCE IDENTIFIER is a sequential number or time of day used to enable an attendance to be uniquely identified.

If a PATIENT is seen by a doctor, sent elsewhere for a test or X-ray and then seen again by a doctor of the same CONSULTANT's firm during the same clinic session, it would be recorded as one attendance - not two.

DECISION TO TREAT DATE (SURGERY) is only mandatory when applicable in the National Cancer Data Set which is when the planned First Definitive Treatment is surgery.DECISION TO TREAT DATE (SURGERY) is only mandatory when applicable in the National Cancer Data Set when the PLANNED CANCER TREATMENT TYPE is National Code 'Surgery' and the FIRST DEFINITIVE TREATMENT PLANNED is classification 'First Definitive Treatment planned'.

DECISION TO TREAT DATE (SURGERY) is the DECISION TO TREAT DATE of the Planned Cancer Treatment with a PLANNED CANCER TREATMENT TYPE National Code 'Surgery' and where the FIRST DEFINITIVE TREATMENT PLANNED is classification 'First Definitive Treatment planned'.

From 01 January 2009, this data element is no longer used in the National Cancer Waiting Times Monitoring Data Set.  It may still be used in other data sets or collected locally if required.

END DATE (WARD STAY) is an ACTIVITY DATE where the ACTIVITY DATE TIME TYPE is 'End Date' of the Ward Stay.

This is the PERSON SCORE for an APPOINTMENT where the ASSESSMENT TOOL TYPE is "Generalised Anxiety Disorder Penn State Worry Questionnaire".GENERALISED ANXIETY DISORDER PENN STATE WORRY SCORE is the PERSON SCORE for an APPOINTMENT where the ASSESSMENT TOOL TYPE is "Generalised Anxiety Disorder Penn State Worry Questionnaire".

The score will be between 16 and 80.

If one or two values are missing from the score, then they can be substituted with the average score of the non-missing items. Questionnaires with more than two missing values should be disregarded.

The classification is required for all PATIENTS who have a Hospital Provider Spell which includes the care of a CONSULTANT in the psychiatric specialties or have been discharged from such a Hospital Provider Spell and are required to receive supervised aftercare under the provisions of the Mental Health (Patients in the Community) Act 1995.MENTAL HEALTH ACT LEGAL STATUS CLASSIFICATION CODE is the same as attribute MENTAL HEALTH ACT LEGAL STATUS CLASSIFICATION CODE.

SECONDARY DIAGNOSIS (ICD) is a classification of PATIENT DIAGNOSIS CODING SIGNIFICANCE.

SITE CODE (OF TREATMENT) is the ORGANISATION SITE CODE of the ORGANISATION where the PATIENT was treated, i.e. it should enable the treating ORGANISATION to be identified.

This identifies the ORGANISATION SITE within the ORGANISATION on which the PATIENT was treated, since facilities may vary on different hospital sites.

The code recorded should always be the national code; if the treatment is sub-commissioned to another NHS Health Care Provider or an independent UK provider, the SITE CODE (OF TREATMENT) used should be the ORGANISATION SITE CODE of the Health Care Provider actually carrying out the work.

Where treatment is sub-commissioned to an overseas provider the Organisation Data Service Default Code 89997 'Non-UK Provider where no ORGANISATION SITE CODE has been issued' is applicable. 

Each ORGANISATION has a unique ORGANISATION CODE. However, where an ORGANISATION has more than one site from which it provides SERVICES, then each site is uniquely identified. These sites are ORGANISATION SITES and are uniquely identified by ORGANISATION SITE CODE. The ORGANISATION SITE CODE contains the first 3 digits of the ORGANISATION CODE with the last two digits being the site identifier.

Example:

For out-patients, ACTIVITY may take place outside the hospital, such as in the PATIENT'S home; in such cases, raising a site code is impractical. Therefore, code R9998 'Not a hospital site (for use on Out-Patient CDS)' would be used in these circumstances.
Note: LOCATION CLASS is used in the Commissioning Data Set (CDS) message to indicate the classification of the physical LOCATION within which the ACTIVITY occurred. 

Use in the Future Outpatient CDS:
If the INTENDED SITE CODE (OF TREATMENT) is not known, this data element should be omitted.

SOURCE OF REFERRAL FOR MENTAL HEALTH is the same as attribute SOURCE OF REFERRAL FOR MENTAL HEALTH.
