A set of FRACTIONS for an individual PATIENT which have been planned and prescribed as a whole. It should be noted that the following all constitute one course of treatment:

a.	Set of FRACTIONS planned and prescribed as a whole and using one or more different machines,
b.	Set of FRACTIONS planned and prescribed as a whole with a period of no treatment during them (split course),
c.	Set of FRACTIONS planned and prescribed as a whole involving machines located on two or more sites.
c.	Set of FRACTIONS planned and prescribed as a whole involving machines located on two or more sites. (exceptionally a patient may be transferred between sites possibly due to machine failure)

If a PATIENT has two unrelated diseases both of which require radiotherapy, each course of treatment should be recorded as a primary course. Similarly if a PATIENT has two primary lesions of the same disease, eg two rodent ulcers, the treatment of these comprises two primary courses, unless the lesions are in such close proximity that they are to be treated together. If during a course of treatment, a PATIENT starts a further course, the second course should be separately identified.

f.	Technical Support; Any treatment irrespective of complexity or beam energy which requires individually crafted items for specific patients such as casts, shells or other individually produced positioning devices or individually crafted beam shapers or modifiers. Includes techniques such as whole or hemibody irradiation that produce major disruption to routine practice with special positioning or dose measuring problems. Also includes teletherapy treatments which require anaesthetic and conformal radiotherapy using shells and stereotactic radiotherapy. Excludes use of thermoplastic materials.

HP190 - RADIOTHERAPY DEPARTMENTS

1. This diagram shows the entity types relevant to RADIOTHERAPY DEPARTMENTS.

2. A RADIOTHERAPY DEPARTMENT contains RADIOTHERAPY MACHINES for treatment, simulation or planning.

3. Each PATIENT may undergo one or more RADIOTHERAPY TREATMENT COURSES each comprising one or more EXPOSURES. A FRACTION is a set of EXPOSURES delivered to a PATIENT in the course of one visit to a RADIOTHERAPY DEPARTMENT. An EXPOSURE may be the single occasion on which a PATIENT is exposed to ionising radiation. For total body irradiation procedures the EXPOSURE may last for several hours.

3. Each PATIENT may undergo one or more RADIOTHERAPY TREATMENT COURSES each comprising one or more FRACTIONS. A FRACTION is a set of EXPOSURES delivered to a PATIENT in the course of one visit to a RADIOTHERAPY DEPARTMENT. An EXPOSURE may be the single occasion on which a PATIENT is exposed to ionising radiation. For total body irradiation procedures the EXPOSURE may last for several hours.

4. The MACHINE OPERATIONAL PLAN is a prospective statement about the time each radiation and simulation machine is to be available for use or intended to be set aside for planned maintenance.

5. The use of each radiotherapy and simulation machine is measured retrospectively in terms of the time the machine was actually available and time spent on planned and unplanned maintenance. This is recorded on RADIOTHERAPY MACHINE AVAILABILITY.

6. The Reference Costs for radiotherapy services based upon Healthcare Resource Groups are derived from RADIOTHERAPY HRG TREATMENT COURSES.
