This patch updates the NHS Data Model and Dictionary as follows:

The data carried in the three Maternity and Children's Secondary Uses Data Sets which support the National Service Framework for Children Young People and Maternity Services Data Sets (Maternity Services Secondary Uses Data Set, Children’s and Young People’s Health Services Secondary Uses Data Set and Child and Adolescent Mental Health Services Secondary Uses Data Set) must be structured in a particular way for submission nationally.The data carried in the three Maternity and Children's Secondary Uses Data Sets (Maternity Services Secondary Uses Data Set, Children’s and Young People’s Health Services Secondary Uses Data Set and Child and Adolescent Mental Health Services Secondary Uses Data Set) must be structured in a particular way for submission nationally. Data items in the files, including the items in the header records, are tilde-delimited (~ character).
Each data set submission file of data is structured in the following way:
- Data Set File Header Row
- Data Set Segment Row(s)
- Data Set File Footer Row

This contains information about the submission file, and is structured as follows, for all three data sets:
The Data Set File Header row is present only once, as the first record in the submission file.
- Data Set Segment Row
This contains Segment type and PATIENT identification information followed by segment content data items, and is structured as follows:
This is followed in the same row by the segment content data items, which vary depending on which segment is being submitted. This structure is repeated for each data record within the file.
- Data Set File Footer Row
This contains some of the same information as the Data Set File Header row, plus a count of all segment records within the file, as a data quality check, and is structured as follows:
The Data Set File Footer Row is present only once, as the last record in the submission file.

The following table is effective from 3rd November 2008 onwards after the relevant section of the Mental Health Act 2007 comes into force, and sets out the relationship between Parts and Sections of the Mental Health Act 1983 (amended by the Crime (Sentences) Act 1997 and the Mental Health Act 2007), and specifies how the codes in Category of Patient, MENTAL HEALTH ACT LEGAL STATUS CLASSIFICATION CODE, Status Of Patient Included in the Psychiatric Census and MENTAL HEALTH ACT 2007 MENTAL CATEGORY interrelate.The following table is effective from 3rd November 2008 onwards after the relevant section of the Mental Health Act 2007 comes into force, and sets out the relationship between Parts and Sections of the Mental Health Act 1983 (amended by the Crime (Sentences) Act 1997 and the Mental Health Act 2007), and specifies how the codes in CATEGORY OF PATIENT, MENTAL HEALTH ACT LEGAL STATUS CLASSIFICATION CODE, STATUS OF PATIENT INCLUDED IN THE PSYCHIATRIC CENSUS and MENTAL HEALTH ACT 2007 MENTAL CATEGORY interrelate.
The underlying MENTAL HEALTH ACT LEGAL STATUS CLASSIFICATION CODE of an Adult Mental Health Care Spell will be carried through a period of Supervised Community Treatment although the MENTAL HEALTH ACT LEGAL STATUS CLASSIFICATION will be suspended during that period.

A FIRST ATTENDANCE is the first or only attendance for the same incident, which may be an injury or occurrence of a condition; a follow-up attendance is a visit to the same department for the same incident as the first visit within the episode. If a PATIENT has a recurring condition, such as epilepsy, or a tendency for joints to dislocate, there would be a new FIRST ATTENDANCE each time that the PATIENT presents with the condition.
A subsequent attendance may not always be a follow-up attendance. It could qualify as an attendance at a consultant out-patient clinic and if so, it needs to be recorded appropriately.

Notes:
This data element is effective for PATIENTS detained prior to 3rd November 2008 when the relevant section of the Mental Health Act 2007 comes into force, which abolishes the Mental Health Act 1983 MENTAL CATEGORIES. For PATIENTS detained from 3rd November 2008, the MENTAL HEALTH ACT 2007 MENTAL CATEGORY data element should be used.
MENTAL CATEGORY may continue to be used for historical purposes for any PATIENT last detained prior to 3rd November 2008 when the relevant section of the Mental Health Act 2007 comes into force. In these circumstances MENTAL CATEGORY will flow in the CDS V6 Type 170 - Admitted Patient Care - Detained and/or Long Term Psychiatric Census CDS, and the Mental Health Minimum Data Set. In these circumstances MENTAL CATEGORY will flow in the CDS V6 Type 170 - Admitted Patient Care - Detained and/or Long Term Psychiatric Census CDS.
MENTAL CATEGORY will be replaced with MENTAL CATEGORY CODE, which should be used for all new and developing data sets and for XML messages.

PATIENT CLASSIFICATION is derived from the ADMISSION METHOD, INTENDED MANAGEMENT and the duration of stay of the PATIENT. The duration of stay is derived by subtracting the date of admission from the date of discharge.
In the case of maternity PATIENTS, the use being made of the delivery facilities is also used in this derivation.
