Home Assessment Visit is a CARE CONTACT.

A visit by one or more members of a occupational therapy or physiotherapy Professional Staff Group Service, to assess whether a client or PATIENT's home requires special equipment or structural alterations. The client or PATIENT whose home is being assessed may or may not be present during the visit.

The outcome of the assessment may affect the need for admission to or discharge from hospital.
