The DA1 form is presented to the social security institution in the state of residence or stay to gain entitlement to healthcare benefits. 

The DA1 entitles people to healthcare under insurance against Accidents at Work and Occupational Diseases (AWOD), when they live or stay in a European Union State, other than the state where they are insured.

Note: prior to 1 May 2010, the DA1 was known as the E123.
