Background:
The NHS Data Model and Dictionary Service are currently working with the Department of Health and Information Centre for Health and Social Care to develop a number of Data Sets.
During the business analysis stage, it has been identified that these Data Sets cover a number of common areas which are either not currently in the NHS Data Model and Dictionary or amendments are required to make the current information suitable for use in these Data Sets.
This Data Set Change Notice adds common data items and associated data modelling that has been identified since the production of data items relating to: Disability, Questionnaires and Person Score, Other Person Observation, Communication, Education, Employment, Activity Offer, Refugee / Asylum Seeker, Accommodation and Sexual Orientation. The changes are detailed in the "Summary of changes" below.
These changes do not need to be implemented in end-user systems until the Data Set Change Notices for the specific data sets come into effect. Other specific items will be added to the Data Sets during development.

A Child Protection Plan is a CARE PLAN.
A Child Protection Plan should:
- assess the likelihood of the child suffering harm and look at ways that the child can be protected;
- decide upon short and long term aims to reduce the likelihood of harm to the child and to protect the child’s welfare;
- clarify people’s responsibilities and actions to be taken; and
- outline ways of monitoring and evaluating progress.

The Children Act 2004 provides a legislative spine for the wider strategy for improving children's lives. This covers the universal services which every child accesses, and more targeted services for those with additional needs.
The overall aim is to encourage integrated planning, commissioning and delivery of services as well as improve multi-disciplinary working, remove duplication, increase accountability and improve the coordination of individual and joint inspections in Local Authorities. The legislation is enabling rather than prescriptive and provides Local Authorities with a considerable amount of flexibility in the way they implement its provisions.
The Children Act 2004 places a duty on Local Authorities to promote the educational achievement of Looked After Children.

The Common Assessment Framework (For Children and Young People) is a standardised approach to conducting assessments of children's additional needs and deciding how these should be met. The Common Assessment Framework (For Children and Young People) is a key part of delivering frontline services that are integrated, and are focused around the needs of children and young people. It can be used by practitioners across children's services in England.
The Common Assessment Framework (For Children and Young People) promotes more effective, earlier identification of additional needs, particularly in universal services. It aims to provide a simple process for a holistic assessment of children's needs and strengths; taking account of the roles of parents, carers and environmental factors on their development. Practitioners are then better placed to agree with children and families about appropriate modes of support. The Common Assessment Framework (For Children and Young People) also aims to improve integrated working by promoting coordinated service provisions.

A Looked After Child (also referred to as a Child Looked After) is a PERSON.
A Looked After Child is a child in the care of a Local Authority either:
- through a Care Order made by a court or
- voluntary agreement with their parent(s) to accommodate them.
They may be looked after:
- by foster carers, or
- other family members.
All Unaccompanied Asylum Seeking Children are also Looked After Children.

An Unaccompanied Asylum Seeking Child is a PERSON.
An Unaccompanied Asylum Seeking Child is a child who is:
- applying for asylum in their own right and
- is separated from both parents and is not being cared for by an adult who by law has responsibility to do so.
A child may move between the unaccompanied and accompanied categories whilst their applications are under consideration, e.g.
- where a child arrives alone but is later united with other family members in the UK
- a child arrives with their parents or close relatives but is later abandoned
- a trafficked child
- one brought in on false papers with an adult claiming to be a relative.
All Unaccompanied Asylum Seeking Children are also Looked After Children.
