An NHS Health Check Programme is a type of Surveillance Programme.
An NHS Health Check Programme identifies eligible participants between the ages of 40 and 74 in England that have not been diagnosed previously with diabetes, hypertension, chronic heart disease or kidney disease. It provides an assessment of their risk of developing these conditions in order to support, reduce or manage that risk though individually tailored advice.
It identifies people requiring an NHS Health Check Assessment to ensure those who are deemed at risk of developing conditions are provided with appropriate information, advice and treatment, in order to improve the management of their health and quality of life.
